These are the required documents for our Biomedical Research Ethics Committee (BREC) application that you may need to do to undertake your research:
- Information for the application form – BREC-Application template below.
- Study protocol – separate document detailing how study is to be performed.
- Survey protocol – survey forms and/or questionnaires to be used in the study.
- The application is made in the name of a UKZN staff member as the Principle Investigator. If you have co-investigators, each co-investigator needs to supply the following:
- Registration form – RIG-Registration-Form (2019) template below (for those outside UKZN to be registered onto the system)
- Signed declaration form – template below
- Signed CVs – template below
- Gatekeepers Letter from both the municipality and institution in which you are conducting the study. Where applicable, obtain letter from relevant governmental department (e.g. KZN Department of Basic Education when working in schools).
- Complete the on-line ethics course – Module 1 and the South African Module (download certificates once done) at the following site: https://elearning.trree.org – PI and all co-investigators and supervisors to do.

If you have a social aspect in your study, then you might also need approval from the Humanities and Social Sciences Research Ethics Committee (HSSREC).
